About the Program: Welcome to the Shifting Mindsets program, a modular assessment program to provide Canadian Primary Care Physicians (PCP) with resources to identify and manage their high-risk post-MI patients, above the LDL-C threshold of 1.8 mmol/L, in accordance with the new Canadian Cardiovascular Society (CCS) Dyslipidemia guidelines.

Program Objectives:

  • ASSESS current practice to identify post-MI patients whose LDL-C is above the 2021 CCS Guideline recommended threshold of 1.8 mmol/L
  • DEVISE a treatment and management plan to get LDL-C for post-MI patients below 1.8 mmol/L
  • IDENTIFY barriers to getting patients’ LDL-C below the recommended threshold

This three-credit-per-hour Assessment program has been certified for up to 7.5 Mainpro+® credits. Kindly note that all components of the program must be completed to receive the credits.

This program has received an educational grant or in-kind resources from Amgen Canada Inc.
